Musculoskeletal Modeling in Spine Surgery – In Silico Experiments

The available musculoskeletal models of the spine are more biofidelic, robust and versatile than ever before. After applications in basic science and ergonomics, the spine modelling has entered the realm of clinical problems and spine surgery. The progress made in modelling subject-specific features together with the rise of statistical modelling methods open the way to in silico clinical trials, promising an attractive alternative to the time-consuming and expensive physical experiments. Yet serious concerns related to model verification and validation, still limited generalizability of the results, and a steep learning curve hinder the wide-spread use and clinical translation of the musculoskeletal spine models. This talk will present the musculoskeletal modelling as a research tool to investigate clinical problems of the spine, discussing recent applications, identifying biggest strengths and most concerning limitations, and indicating directions for the future use in spine surgery.

Short bio:
Dominika Ignasiak is a Group Leader in Musculoskeletal Biomechanics at the Institute for Biomechanics at ETH Zurich, Switzerland. Her work is focused on exploring biomechanical causes of spine pathologies using musculoskeletal modeling and other computational methods. Her main research interests include spinal alignment and balance, adult spinal deformity and proximal junction disorders, and sarcopenia. Ignasiak is recognised for the first AnyBody spine model with fully articulated thorax and ribcage she established, used now by several research groups around the world. Working in medical device industry, she developed a novel application of this model for preoperative planning (US patent application), which is currently being validated in a multi-center clinical trial she designed and initiated.

Anterior Deformity Surgery. What we need to know for the Future

Short bio:
Jürgen Harms is an internationally recognised specialist in the field of spinal surgery. He pioneered advanced surgical techniques in scoliosis surgery, tumour surgery, transoral surgery, the TLIF technique or dorsal fixation of the atlanto-axial complex, which are practised worldwide today. In addition, he was the developer of the polyaxial screw-rod systems, which are considered state of the art today. His many years of successful exchange between medicine and technology also led to the development of new implant systems for spinal surgery, including the Harms titanium basket named after him. For more than 20 years, Harms has endeavoured to highlight the importance of ventral surgery for the treatment of spinal deformities. The possibilities and results are comprehensively presented in the book "MODERN MANAGEMENT OF SPINE DEFORMITIES", published together with Bob Dickson. He is a speaker at international medical congresses and supports various research projects as a consultant. The Harms Study Group is considered one of the most successful study groups with more than 100 trend-setting publications and lectures on spine surgery.

  • "Spinal Deformity" with Ibrahim Obeid. Dr. Obeid is a spine surgeon and serves as a Consultant in Spine Surgery in Bordeaux University Hospital Spine Unit. His clinical interests involve adult and paediatric spinal deformity surgeries, revision and very complex deformities including complex cervicothoracic spine reconstruction and navigation assisted spinal deformity surgeries.
  • Full-endoscopic surgery - The next step in minimal-invasive spinal decompression with Christoph Siepe. Prof. Siepe is Head of the Department of Spine Surgery at the Schoen Clinic Munich-Harlaching and an expert in the application of the minimally invasive, endoscopic surgical technique in spine surgery.
